This is the letter that you would send along with your identity theft report, affidavit, and police report.  The affidavit and identity theft report are one in the same.  If this information is showing up on all three or even two credit reporting agencies, it is only necessary to contact one for investigation.  Upon successful completion and clearance of your name the credit reporting agency that investigated the claim will report to the other two credit reporting agency.
